Well, I dunno--I think it's pretty normal for him to notice them and talk about them. When I change/shower/whatever with my DD around, she is always commenting about whatever she sees--"mama's bum", "mama's tummy is bigger" (I'm pregnant), and stuff like that.
Maybe it's because he calls them "milks" that it makes you think he's thinking about nursing? Toddlers and preschoolers are all about talking about body parts and potty terms.
I do think having him reaching in your shirt is a nuisance and probably just an old habit that he doesn't really know why he does--except that mama is soft and cuddly, probably. I would just redirect him to some other activity when he does it and I bet he will stop fairly quickly. I honestly don't think he's thinking about nursing that much.
