I decided to not vax after DS2 was about 1 and before we moved up here after DH got out of the Army. The religious waiver form states that:
"Immunization conflicts with the religious tenets and practice of a
recognized religious denomination of which the student is an adherent or
member or immunization conflicts with the student’s personal and
sincerely followed religious beliefs."
(Signature of Affiant) (This would be me, right?)
Has anyone had any trouble with this? I will homeschool if I have to but would rather not.

I fully vax, so I can't help you there. Is your form for elementary school or the pedicatrician's office or daycare or what? It might help another mama help you with your question.

The form is a generic one, I believe, as I got it from the hhs.state.ne.us site that posts the Nebraska Immunization laws.
I am in Norfolk, now but we came up from Alabama after living in Germany for 4 yrs. My DH and I both graduated from high school here in NE so we're not new, just coming back home. :happyclap:

Not in Nebraska, but when we lived in New York and DS attended daycare while I was finishing college I had to write a note exempting him from vaccines. I just wrote my own letter, basically stating that my sincerely held religious beliefs prevent me from harming my child and after careful research and discussion with his pediatrician, I felt that the vaccines could do more harm than good. Never had any problems with it.

You shouldn't have any issues. I have a few friends back home that select vax so I would think they are in the same situations as you. Good luck.

Thanks, mamas! I had originally looked at selective vaccinating and even had a schedule worked out with my pediatrician. He was all for it. However, when I looked at what was required for school and what I didn't want the kids to have there wasn't an option for it. All I could find for selective was for the Dr. to sign off saying xyz would be medically harmful for xx child and, of course, the dr. wouldn't lie and sign a form like that if it wasn't true. So, I figured it was all or nothing so I chose the nothing.
