I'm pretty sure we're not supposed to modify at all. I personally don't mind putting on a pair of undies underneath but granted I've only had short/light visits from AF since DD is 95% BF plus I have the Mirena IUD. So I have yet to try heavier flow MC but I imagine it will be the same. Although I plan to use a Lunette cup when I start getting "real" visits from AF again.
