My daughter was a home water birthed baby and I just put a plastic bowl on the top of the water and put the placenta in it to float around in while I held baby down in the water with me. Worked great!

I guess the obvious answer would be to stand or kneel in the water and have someone else hold the baby on the surface of the water. I agree it's only really effective if the baby is lower than the placenta.
