Do the religious form and don't think anything about it. They don't have to know what vaccines you do/don't do...just give them the exemption form and don't even worry about giving them the vaccination record that you *do* have (that's what I'd do anyway, I'm in Illinois). Our pre-k is in the public school and those things aren't required for us...recommended, yes...but not required. They "require" the chicken pox for kindergarten and recommend the flu shot in our school for K as well.
