My first was with an epi, and I pushed in the semi-reclined sitting position.

My second was without pain meds, and they had me start out semi-reclined and sitting while holding my thighs. My DD was posterior and the pain in my back was unbearable in that position, so I ended up squatting on the bed to deliver her. But my MW also told me I could start pushing before I felt the urge (I was fully dilated/effaced) because she said it could take hours before I felt the urge due to baby's position. At the time I just wanted to get things over with, but the pushing was so incredibly painful that I think in the future I will definitely wait for the urge since it goes so much easier then - and she may have turned to a better presentation if I had waited longer.
