I donate my extra milk to a milk bank. I started pumping when DD was 3 days old, as she was refusing the breast and losing too much weight (although she is a breastfeeding champ now and gaining like crazy). Because I was constantly pumping for the first few weeks of DD's life, I now produce more than she needs. I nurse her when we are out, but when we are at home, I pump one breast while she nurses on the other. I am able to collect quite a bit of extra milk. It's very rewarding to donate something so special to sick and premature babies who might not otherwise be able to receive breastmilk. I believe that every baby deserves breastmilk and encourage you to at least try to pump for your sister's baby. If it doesn't work out, at least you tried!
