i know she already has a sling comming, but i'd go for a mei tai or something a bit different so she could get the whole spectrum of babywearing...

a tub of shea butter, for her and the baby

hooded towels...they are the thing that is never necessary, but NICE to have

or a good nursing pillow. i was given a boppy and it's didn't work all that well for me until dd was a good bit bigger - i've heard good things about my brest/breast friend. or a nice manual pump like the Avent Isis (esp. if she isn't going to need a fancy Medela or something )

I almost always get nursing sets (necklace and braclet) for friends but for my sister I would get something bigger (provided you have the funds) maybe a housecleaning service for a month or dinner delivery service for a month or something like that.
