I had the same exact thing and it was in part due to my blood pressure being abnormal at night and my liver function. I see an acupuncturist regularly and she gave me an herbal supplement to take that helped a ton.

Look up cholestasis like they said. I had this and started with itching all over, no rash. I got a "rash" from itching. Usually the itching is worse at night and on your palms and soles of your feet. Hope that's not it, but if it is it is treatable. Make sure you ask your OB about it, please...it can be dangerous if not treated.

Actually, sorry to say this, but this does NOT sound like Pupps, it sounds like Cholestasis to me. I diagnosed myself with this because of the lack of rash. The itching on the palms of your hands and bottoms of your feet is what clues me in. My doctor kept telling me I had pupps and to take benadryl, but if I had listened to him and not checked myself in the hospital, the specialist who helped me said I could have lost my son in utero. I am not trying to scare you, but not many people know about cholestasis and even my dr. said it was rare. I BEGGED him to check me for cholestasis and when he finally did, my liver enzymes wer 4x the normal human limit. I think you really need to tell your dr. you want your liver enzymes tested. They have a quick blood test and if it is elevated, they can send it off to have it further tested for cholestasis. It is really important that you rule it out because it does carry complications of stillbirth and I almost needed a liver transplant. The only thing that clears it is delivery so if you do have it, you may need a high risk specialist. I was referred by my o.b to a specialist and he delivered my son at 35 weeks. My test came back positive after I delivered my son.
I wasn't able to sleep. I would get up with a fork and scratch the bottoms of my feet. The only rash I had was blood pricks from scratching, no actual rash, which is usually pupps. Please pm me if you have any more questions.

Oh, and I had itching in ALL of the places you describe, exactly.

Yeah, I don't think it is from the skin stretching, if it is happening on the bottoms of her feet, she really needs to assume it is something else.
