Nope, my kids are 1 and 3 and neither has ever had a birthday party aside from cake and ice cream and family. When they start school and have friends then I might start throwing parties.

We don't do other holidays so I like to give gifts and make a fuss over them on their birthday. We have church friends that come and some family too. My DS1 is turning 3 Sunday... ;(

Yes we do have a party every year. However, it's not usually more than grandparents (plus great-grandma), my uncles, and my sister and her family. Maybe have a few friends this year too but that's about it.

Yes, although we limit it to family only for the first 3 years...4 years for my youngest!

We don't throw parties anymore, we treat birthdays as special family days. DH takes the day off and birthday kid gets to pick all the meals including one eaten out if they want, they get to pick an activity (build a bear, bowling, tea @ tea house etc). I also decorate after they go to bed the night before so when they wake up there are streamers and balloons, decorated chair and special birthday hat. They love it much more than a party that only lasts a few hours!
