Pack and play, bouncy seat and exersaucers are the three big ticket items that have seen tons of use.
Infant seat/stroller
good quality carrier (not the ones sold at Target)
swing
high chair that straps onto the regular chair

Good quality breast pump

Small stuff
~different types of bottes/pacifiers. Babies can be picky.
~swaddle blankets
~extra crib sheets/matrass pad
~pack n play sheets
~toys for different ages
~different soaps/lotions
~baby bath tub.--Ours fit over our sink in the kitchen which was awsome for itty bitty
bath and then the older baby could sit up in it.
~baby towels or smaller bath towels
~contaiers for snacks
~sippy cups (again many different brands and stages)
~baby spoons and toddler silverware
~diaper bag
~car seat cover for winter
~high chairs/seats that hook onto a table. loved it for travel and we also used in our
our island at home

I think registry items are tough because babies have such different personalities. The handful of things I was the most excited for are the things I've used the least and the things I didn't register for are the things I needed the most/borrow/bought from consignment!

The advice about medicinal stuff and 6+ month stuff is excellent. sometimes it's about finding your child's personality first and getting the 'stuff' to accomodate later.

That being said, if I knew then what I know now ..
GOOD quality Swing
Swaddle Blankets
Boppy
Sling/Carrier

That pretty much takes you to 3-4 months of age

Nursing pillow/boppy
swing
playpen with bassinet (can't fit crib in our room, and I'm not dishing out the money for a bassinet if I can just get a playpen instead!)
