Rather than leaving the wash cloth on his forehead, take it and get it wet with warm water. Then rub it over his forehead and arms, etc. The moisture from the washcloth will evaporate off of his skin taking some of the heat away.

But, with the symptoms you described, I would take him to the doctor. High fever, lethargic, decreased urination. These are all potentially signs of dehydration.

Update: Motrin broke the fever and he seems to feel fine this morning though he's hovering right at 100. I am keeping him segregated though as I posted several days ago about hand, foot, and mouth going around my church. I am PRAYING that this is my HFM. My mon is flying out on Wed for 10 days of what she's calling 'Summer Fun' and it will not be fun if my kids are all dealing with HFM.
