There are a bunch of explanations why your dd could have had some vaccinations - maybe you don't object to ingredients in some, maybe you just started following your religion after she got some vaccinations, etc.

Just fill it out and they shouldn't ask. Although I did have the stupid nurse at the hospital DEMAND a verbal explanation when I declined the hep b for DS (even though she doesn't need an explanation since there is a form for me to fill out). I just told her I was afraid of zombiepocalypse.
