Smegma is a secretion that is constantly forming. It will always be moving anything(skin cells, etc) out. His foreskin doesn't have to be pushed back, it will take care of itself.

I thought smegma didn't show up until puberty. I'm new to the intact thing, but my 6yo doesn't have smegma, I don't think. Then again, he's very private and I haven't seen his penis since I changed his diapers at age 3. If it's red and there's white stuff, I'd guess yeast infection.
