What would cause foamy spit up?

I've been pumping and feeding my now 1 week old my bm. Well last night when I gave her the only oz that I could get out, I gave her 1/2 oz of formula like I had been doing if I couldn't pump anything. She was always fine before but last night she started spitting up everything and then was gagging on this foamy phlem Two hours later when she was hungry again we tried to give her the bm I pumped but she spit it all back up. Could there be something in my bm causing this? She was fine in the middle of the night when I gave her the formula. And she refused to drink my bm this morning too.

From the title I was going to say poor latch and too much intake of air. But then I see you are pumping. I have no advice there. But I would suspect it is something with the formula. Just my 2 cents.
