Red face Going to try one more time and need advice

I'm pregnant w/ my fourth child. DS1 was born at 36 weeks, DD1 was 35 weeks, and DS2 was 32 weeks. I breastfed ds1 for 3 months until i developed mastitis so severely, I was hospitalized for 3 days. By the time I got out, my milk was depleted and ds didn't want to nurse. With dd1, my milk never came in and the dr's said it was possibly from damaged milk ducts when I had mast. With ds2, he was in the NICU for 2 weeks, so i had to pump. I was able to get the colostrum out just fine, but then I had problems getting him to latch. By the time he came home from the NICU, I was able to bf for 3 more weeks until he didn't want to nurse anymore. Each time I've tried, my biggest complaint has been horribly sore and cracked nipples. I know it's probably from poor latching, but I'm thinking there may be something else I'm doing wrong as well. I think I remember reading somewhere that I can "prep" my nipples leading up to the day I deliver. Does anyone know anything about this? Is it just using nipple cream to condition them or what? I really want to stick w/ it longer this time, but Im dreading that horrible pain. Any other advice you have to help me get ready to bf is greatly appreciated.

That business about prepping your nipples is an old wives' tale. Please don't do anything rough to your nipples before your baby is born! My MIL told me I should do that before my DD was born--it seems like a very common, though misguided, idea. One of the mamas at my LLL talked about how she had nursed a toddler all through her pregnancy, and she still had some soreness when her newborn nursed for a while--those little newborn mouths can just do that, until they grow a bit and get the hang of nursing, too.

But I would recommend that you line up some really good professional support from a lactation consultant--and not just anybody, but an IBCLC, who is certified. Hospital nurses can give you such conflicting information and many of them aren't adequately trained in breastfeeding. Find someone before your baby is born and have her come see you and your baby as soon as he/she is born. She can even come see you multiple times. The help is so important.

Also, have you gone to LLL meetings? LLL leaders can provide peer support and answer questions, even over the phone. You can get so much support from other mamas at the meetings.

You can have lots of comfort measures available at home. I had some flax-filled fabric pouches that I could heat or cool and then place on my chest, depending on what kind of comfort sounded the best. I also used some of those gel pads on my nipples when baby wasn't nursing. Oooh, those are nice and soothing. A good nipple cream is helpful, like lanolin. Also, letting your breasts get as much air time as possible is helpful, too.

I was very fortunate--I had very little nipple pain, and it only lasted a couple of days.

You can start using lanolin before birth... It does help. But to 'condition' your nipples is a bad idea!!!

I don't have much outside of what the PP said... GL! You can do it mama!!!

check out Dr Jack Newmans site for latch videos. The smallest adjustment might make the world of difference!

Definitely get in touch with a LC! Did you see one in the hospital with your other births?

Thanks for the support mama's! Catherine, that was all wonderful advice. How do I figure out if LLL meets here in town? I've never been to a meeting. The hospital does have a special lactation nurse at the hospital. I'm not sure if she's certified or not, but I"m def going to check now. A "specialist" came to spend time w/ me when ds1 was born, but I didn't feel like I received very good info, so she may not have been certified either. Where do I find out all that information?

I agree 100% with the posts above. With your history of preterm deliveries, I would stay away from those nipples! Nipple stimulation can throw you into labor. With your past experience, you probably don't want to do that again if at all possible.
