Vaccination isn't just an individual medical treatment. It is a public health issue. Agree with or not, vaccination is of particular importance to the larger community.
I delay my vaccinations, so my 4 year old is up to par. My 18 month old has never been vaccinated. Their pediatrician has never asked me to sign anything one way or the other. Not one thing. I'm not sure why you think I should be signing off on all of this information, but I'm willing to bet they are not legally compelled to have me sign anything.
