21 month old wetting through - wet pul?

My daughter has been SUPER wet several times lately, so much that the pul is actually wet. Usually this is after a nap or in the morning but it happened this afternoon (picked her up and my whole shirt was wet!)

Is thus a sign of a problem with my pul or just the limitations of a diaper? Some of these overnights have been in very new bought new by me pockets.

The prefolds I typically stuff with are getting pretty worn
-- should I replace these to get better absorbency? I've added doublers for day and it's helped, I just had some already stuffed without that DH used.

Thanks for any input! I don't have stinking or repelling (as far as I know) and it's not every diaper.

Are you using just pockets? I'm not familiar with them but have heard others having issues. I'd say add absorbency and see what happens. Maybe hemp. I love geffen baby doublets for my toddler
