I just wanted to post back that we got this resolved and are now at 99% daytime potty success! I couldn't have done it without good old Dr. Sears. It seems to have been the confluence of physiological issues and emotional withholding (that terrible constipation-trauma-withholding-constipation cycle). We started amping up the fiber in DS' diet (which is tough since he refuses fruit), decreased dairy (plus switched to lactose-free milk... just in case) and also added a splash of flax seed oil to his morning milk. Within a few days he not only started pooping on the potty, but taking himself there and doing his business totally independently! The only thing left is wiping -- he even attempted that once, but for now I think we'll keep that job with mom :-)

So far he's been poop accident free at preschool for over a week and going at home every day or every other day. We're so proud of big boy and so is he!
