maybe pair nursing pads and a near by restaurant/fast food gift cards...and make up a cute note about something for feeding the baby and something for feeding the mama (that is, if you know she is planning to breastfeed)

That's a good idea. She bfed her older one until just past a year, so I assume she will this one as well. I have no idea where they like to eat... I guess anything really. I am kinda tempted to make her x pairs of pads and make baby x bibs out of the same fabric. Bibs are pretty easy and fast.

I think the breast pads idea is fantastic! Also, one of my favourite gifts were some flannel burp cloths a friend bought at an art & craft sale. So useful, soft and cute!!

Otherwise, I would suggest an amber necklace for baby, a nursing necklace for mama, a charm bracelet for baby, a Sofie le Girafe or even better, one of Sophie's mushroomy cousins (if you can find them at a local baby store or Barnes and Noble for $20).

You can make them from any light fabric, either already colored or dye with kool-aid. Old gauzy curtains or satin sheets from goodwill work great, you can make several sizes and serge the edges or add trim. Add a strip of coordinating trim along one edge with about 6" of extra on either side so it can be tied on as a cape, apron, skirt, or veil!

A fake "tag" blanket is always quick and easy. Just sew a bunch of ribbons into the seam of a small blanket that the lo can cuddle and play with the ribbons.

If you wanted to do something for the older sibling you could make a blanket for a doll or stuffy that matches the baby/mom gift.

That sounds like a really cute idea with the tag blankie. Maybe pick up an inexpensive baby doll for the big sister & make a matching blanket for her new baby, too.
