I have an LG HE and although my routine is involved and complicated sounding it is the only one that works so far.

Rinse and spin
Rinse and spin/no spin
cotton wash stain cycle water plus with gain liquid a little below the one line and a splash of bleach.
cotton wash warm/warm water plus with a splash of vinegar to help rinse out bleach
rinse and spin no spin
rinse and spin

Diapers come out soft, clean smelling and stain free. AND they don't stink when peed on. A big plus in my book. Because of my washing routine I wash covers seperately most of the time. We just use mostly flats and mf inserts with a few random pfs and stay dry suedecloth socks.

Non-steaming samsung he fl'er (one with wool cycle):
27 min quick wash to remove the excess urine and bm (rarely add it, but sometimes some oxy here)
sanitary cycle with either liquid tide or drops and 1/4 cup bleach.
60 mins in dryer on high heat.
