A decent carrier. With DD we had a kangaroo sling she lived in. This time we have that, an Ergo with infant insert and a moby. I got so much done hands free. I loved the halo sleep sacks for winter and fall. We did use the light weight ones for spring. A bottle sterilizer if you will be hand washing bottles. The nosefrieda nose suction thing, a forehead thermometer and rectal (rectal is more accurate for when you need it to be 100% but the head one is much easier and can be done when asleep).

And a variety of cloth safe diaper creams. Even babies r us now has the bum genius diaper sprayer, I'd add that if you don't have one and a bunch of wet bags and a pail if you need one.

We had a bottle warmer and never used it. DH realized that DD was totally okay with room temp :-) And DD hated the bouncy seat. I wouldn't invest in a swing or bouncy seat - borrow, or try out at someone elses's house to make sure YOUR LO likes it first.
