that HAS to be reported and quite frankly I would sue the pants off of him but at the first mention of such a thing we would have been out of there.

Completely unacceptable. That poor baby had to have been screaming. I would be on the phone to the police before leaving

I have 3 girls and I have never had a well baby visit where baby's diaper is even off! WTF?

Even if there's a tiny grain of truth in that story, I'd guess it had more to do with labial adhesions than the hymen.
