Ugh. The school said no exceptions on the well child visit! Ridiculous!! So I spent the morning calling a million places. The Walmart clinic wouldn't see him until he turns 5. Urent care wont accept insurance for a physical since it isnt "urgent" Most drs can't get him in until 2 weeks from now, since he isn't "sick" *sigh* one dr wouldn't even schedule him an appointment since I don't have his records from our previous home! How will they know he's "well" with out a history? (<---insert sarcasm here). Now I'm on a waiting list for a cancellation. If I don't get the paperwork in by the 26th he looses his spot!

I will give it to Friday before I go to one of those, since Walgreens etc, won't take our insurance for a physical. But it will be. Last resort. I also need something signed by a dr so they can admin benydryl if he gets into peanuts, I don't think a clinic can sign that??? Idk..

My DS just started preschool (in CA) and we had the same form. We almost never go to the Ped, but I called the office and they were familiar with the form. We faxed it over, they filled it out and faxed it back. We don't vax so basically they just filled out when his last appointment was and his info, then we filled out a vax exempt form to file with it.

my kids (for public high school from homeschool) have never needed a physical for school. They did need one for school sports, but I was ok with that.
