In this day and age, kids have different last names all the time. I would just send her with a birth certificate, and nobody mentions that she isn't her daughter.

But, I would send a notarized letter consenting for treatment in case she has to take her to an ER.

I haven't read the responses, and our situation is a little different, but I thought I'd chime in. We live with my parents in CA, and my mom told me a couple months ago that she was considering bringing our youngest (2.5) with them to ND for a long weekend. I'd never been away from him for more than a night. I knew he would be fine, but I knew I'd miss him. Before I officially gave my permission, my mom said they'd already bought him a plane ticket (I know, annoying to some people, but I wasn't really bothered). Anyway, they did take him to ND last week, for four nights. And DD(4) was a tiny bit jealous, but mostly she was happy to have DH and I to herself for a few days. In the end, it was a really great experience for all of us. DS got to spend time with his great-grandparents, and he had a wonderful time. And really, I didn't end up missing him as much as I expected. But I sure was happy when he came home!

So, my advice is that if you really do trust then people your DD will be with, then just go with it and make it fun for everyone, even your kiddos who will stay home.

I miss my kids too. But, it isn't about ME. Its about THEM!!!! And they have a great time.
