Those CDC recommendations don't account for preemies' unique needs. The mama who said her DS is getting rotovirus at 3.5 months is probably vaccinating according to his adjusted age. If he's already started the series, he would have been younger than 14 weeks adjusted age and probably would have been younger than 14 weeks actual age.

FWIW, I would do DTaP and rotovirus.

I'm delaying the DTaP until after 6 mo. I've read that the Pertussis vaccine isn't very good at establishing immunity before then. Plus I had a bad reaction to that vaccine . . . granted it wasn't the acellular version but it was serious enough that I don't want to take my chances with DD.

The effectiveness of the pertussis portion of the DTaP is not high enough for me to give that vaccine, and the potential reactions are higher than many of the other shots. Given your circumstances I would hold off on shots until after you stop EBFing. That strengthens the immune system more than anything else (IMO).

He was 10 weeks actual when he got his first dose.
