Agreed, I think it looks like eczema that has gotten irritated from something. I'd try hydrocortisone on it twice a day, and keep it covered with a heavy moisturizer the rest of the time - Eucerin maybe, or something similar. Not the most natural options, but that always cleared similar rashes up quickly for my son in a few days.

Puzzled here too, but every time we've encountered some sort of rash, infection or eczema, oatmeal baths have helped.

Looks like a reaction to me. Maybe try a staydry liner with those cotton prefolds? I agree with the hydrocortizone idea. And oatmeal/chamomile baths.

I agree with eucerin and cortisone too, I hate using it on DD but it is the only thing that clears her eczema... She is 4 and has been battling eczema on her bum all winter... Once it is clear I can stop the cortisone, and maintain with aquaphor, but every time she spends a few days with dad in charge and she doesn't get the aquaphor, we end up back at square one.

I would do two baths a day with plenty of moisturizer and as much naked time as you are able to pull off until that goes away. If baby is on solid foods (not sure how old you said she was) I would try an elimination diet to see if it's being caused by a food allergy (food allergies can be weird like that). Has your baby been in a swimming pool or anything like that lately? It could be some kind of bacterial rash.

My DS gets that from his own urin. If he is not changed right away he gets a nasty amonia burn. I couldn't CD him and when he was potty training it was nasty (preschool wouldn't change him right away if he had an accident) he still gets burns at age 5 if he pees the bed and doesn't wake up. Those are few and far between. I have to keep him dry and away from his own urin.

You mentioned switching from pockets to prefolds; do you think the wetness against her skin is the source of irritation? My son always did better with stay-dry fabric next to his skin.

My DD isn't as sensitive but these kinda rashes is why I went to disposables at night.
