How high do you set the dial on your Medela PISA?

I can't turn up my Medela PISA more than halfway on the dial before it starts to hurt. Is that normal? What do you set yours at?

I can actually go all the way without it hurting. Everybody is different. If it hurts, then turn it down. As long as you're pumping milk, you're fine.

Yeah I know that I shouldn't worry - everyone is different. But I always have this feeling that there is extra milk in there that isn't coming out! And yesterday I had proof - even though I pumped 4 times at work by the time I came home my left breast was engorged with hard spots and very painful. The left one is the slacker side too - so even though I came home with 10oz of milk the left side had produced only a little of that.
I did size up to the 27mm flange but that only seems to help a little. In fact, sometimes with the 27mm flange my nipple turns white around the base.

Try breast compressions while you pump. That should help.

Sounds like maybe you need to size up your flange. The pressure setting shouldn't make a huge difference. I couldn't turn the PISA past the low end and still pumped milk. I feel like my nipples are being yanked off if I go higher.

I don't like turning mine up very high either! The highest I go is half way, but I am producing enough to keep up with my little one so I don't worry about it too much. I do wonder if I would pump more if I turned it up higher...

I turn mine all the way up. It doesn't hurt and I only get to pump twice while at work.

i only go about 1/4 way. some days higher, i turn it up until it's uncomfortable - then go down a notch...i don't really have a specific setting
