It's kind of funky because even if you have a contractor at your house sign something like that there's still a chance your liable. If I had a trampoline or pool a lot of kids hung out at I would definitely contact a lawyer. By a parent simply saying "yes my child can play at your house"...doesn't protect the property owner for sure.
