I'd get the medium. My ds is 20 pounds and still has that fat baby belly and he wears the medium HHs. I can't pull the aplix tabs to the middle, but the thighs and rise are right. I can put my 26-pound toddler in a medium in a pinch, although she usually wears larges--but she's pretty skinny.

I know what rise is, but do you measure rise on a bare bottom or a diapered bottom?
