what is difference between flat and prefold?

Technically a flat is one flat single layer piece of fabric, you can fold it to the size and shape you want.
A prefold comes in a few sizes and has more layers of fabric in the middle than the edges. Such as 4 layers on the sides and 8 layers down the middle. This is to minimize any folding that would need to be done.

Not necessarily. The prefold is called "prefold" because someone took a flat and "prefolded" it for you, and then sewed down the layers. You can fold a flat to achieve the same thickness of a prefold (most flats are approx. 28" x 28"). I have both in my stash for the new baby coming so we'll see which I prefer...

Generally speaking yes. Only the smallest prefolds would be less absorbant than 1 flat. A traditional flat has larger dimesions than a prefold though, 27x27", to be folded down and is absorbant enough for frequent changes. People do use 2 flats or a doubler on larger and heavy wetting babies.
