What CDing goofs have you made?

I've seen quite a few threads about DH's goofing up (putting wool over pockets, forgetting inserts, etc), what I'm wondering, what goof-ups have YOU made?

I'll start by saying when I first started CDing, I bought a bunch of Pocket Change seconds that I knew weren't coming with inserts. I went on ebay and bought what I thought were inserts and stuffed my pockets with them. A good week went by, and for the life of me, I couldn't figure out why my dipes were constantly leaking. After a little research, I realized my "inserts" were actually fleece liners

thankfully I have had a lot of guidance.but I did put Bourdreaux's buttpaste on my DD because she had a horrendous rash and it stained some of my dipes
I live in an apartment and cant get a clothesline up on my porch so i can't try to sun them

One of my first posts on DS was asking if I could remove the inserts from my BGOS dipes and change the insert but reuse the actual diaper multiple times. I had been using prefolds and covers before I started using the BGOS, so I thought maybe I could use the BG diaper like a cover and the insert like a prefold. Everyone said that I probably shouldn't, so I don't!

Another one of my firsts posts was because my wipe solution was making DS scream out in pain. Turns out it had witch hazel in it, which was stinging his skin even though it didn't sting when I tested it on myself.

I have put fleece shorties on over his BGOS before, but it was on purpose because he can un-velcro and totally remove his BGOS and I didn't have any of his pants on hand.

One day, and I still don't know how i did this, I put DD down for her nap w/out a soaker on. She had a GM and a pair of pants... but I forgot the cover... needless to say I had to change the sheets and the pants...

I've forgotten to put a cover on a handfull of times....accidently dropped a poop in the washer and had to fish it out (by far the grossest thing yet ) and I've tossed non machine washable wool in the washer.

i havent completly screwed up anything yet... but i only just started. dh has messed up a few times already.
he forgot the cover over a pf, used the unfant insert with a bg, (she totally needs the reg one) and used a bg, which are supposed to be for overnights, and used it during the day, as said above. lol
he'll get it. at least he will try whatever i throw at him.

Ii have bought pockets off of Fost without insert and was so excited about getting them I forgot to buy inserts until they arrived and I didn't have any.
I also bought some prefolds and forgot to ask what kind they were Indian or Chinese It wasn't listed and I needed them so bad I guess I just jumped at the sight of them. I asked the Mama after I bought them and she said the were what I wanted. but when they got here they were the roughest things I have ever felt. I still have them. I can't even bring myself to sell them to someone else.

I can't really think of anything off hand. The worst DH has ever done was put a fitted on inside out.

I put a Happy Hempy pocket fitted on without a cover before---under pjs overnight, lol. OOOPS! I thought it was just a pocket.

I also had a bunch of leaks in the beginning because I wasn't careful enough to make sure none of the prefold was peeking out of the cover. I had no idea what I was doing back then!

When I first got my diapers, I put a couple of backwards at first. I kept wondering why it was so difficult.

I still have trouble remembering to secure the Velcro before washing.
